The Connecticut Age Well Collaborative is an initiative of Connecticut Community Care.
Connecticut Community Care helps people live their best lives at home with meaningful connection to support and services right around them, in their communities.
Connecticut Community Care’s more than 300 care managers are partners, solution seekers, navigators, and advocates for individuals and their families. They listen, assess, and help people achieve their goals by developing customized plans of care and delivering high-quality care management and transitional support. The team shares information about critical resources, provides connection to quality services and supports, and follows up with ongoing guidance and further engagement as life circumstances change.
Understanding that thriving at home requires Connecticut communities to be designed and ready to support residents across the lifespan, Connecticut Community Care founded and launched this Connecticut Age Well Collaborative initiative.
